Modeling Contest Application

To enter our model contest do the following:
1) Print off the below form and fill it in.
2) Send in the following:

  1. the below form
  2. two photos (one of a headshot, one of a full body) - Note: copywritten photos will not be accepted without a photographer release form.
  3. an entry fee of $20. to process your application. Please note it may take up to 30 days to receive your free copy of Prom Magazine.
  4. a photographer release form if necessary for copywritten photos.
  5. Send photos, application and fee to Prom Magazine Inc. 501 S Lake Avenue Ste 102, Duluth, MN 55802
  6. contest ends May 31, 2009.

I, ______________________, do hereby acknowledge the following:
I am voluntarily entering a model contest where the judges decisions are final. I am over 18 years of age, or I have had my
parent or guardian sign this contract authorizing their approval at my entering this contest. The undersigned
authorizes the use of their submitted photos to be used by Prom Magazine Inc., DeBora Rachelle Inc., or any of their affiliates for use of these photos in their magazine, on their website, in any print ads, or anywhere else they see fit.. I authorize Prom Magazine Inc., DeBora Rachelle Inc., or any of their affiliates to alter these photos if necessary as they see fit. I understand that any photos taken of me by Prom Magazine Inc., DeBora Rachelle inc, or any of their affiliates may or may not be used in print. I have sent non-copywritten photos or I have included a photographer release form provided by the photographer who took these photos. I understand that if I am chosen as a winner that I am responsible to provide my own transportation and lodging costs to the photo shoot. I release Prom Magazine Inc., DeBora Rachelle Inc. and their affiliates from any liabilities or wrongdoing pertaining to the use of these photos. If I am a winner, I release Prom Magazine Inc. from any liabilities or wrongdoings that may occur at the photo shoot or any other means such as transportation. I understand that the judges decisions are final and that Prom Magazine Inc. is holding this contest to provide an opportunity to a child or adult that may have always wondered what it is like to be a model. Models are expected to view this as an addition to their resume and are expected to be on time when the date is designated. If the winner chooses cannot attend the photo shoot on the scheduled date (determined by Prom Magazine Inc.), or misses the photo shoot, the runner-up will be given the place of the winner and the winner forfeits any claim to the title or eligibility of this contest. In case of a tie, more than one winner may be chosen and the same prizes will be given to all winners.
The photo shoot date will be predetermined by Prom Magazine Inc. and notice will be given to the winner one month in advance of the shoot. If the winner is unable to make the photo shoot date, he/she will step down and give their winning title to the 1st runner-up.
